More than 150 million Americans rely on employer-sponsored health insurance, but the system is crumbling. Fewer than 60% of smaller firms (200 or fewer workers) now offer coverage — an all-time low — as soaring costs push small businesses to the financial brink. Large companies still largely offer insurance, but rising premiums and high out-of-pocket costs mean many workers cannot afford to actually use their coverage.
